TEAS form previews

Submit only if TEAS is unavailable

The Trademark Electronic Application System (TEAS) form previews are provided for two purposes:

1) To view the questions on the form to prepare for completing the online form

2) To print and submit when you are unable to file electronically due to a system outage

Only use the following TEAS form previews to submit if TEAS is unavailable and you meet the conditions explained on the submitting documents to the USPTO when unable to file electronically webpage. If you don’t meet those specified conditions, all trademark applications and documents must be filled online using TEAS.

Notice: Section 508 of the Workforce Investment Act of 1998 requires that all U.S. Federal Agencies make their websites fully accessible to individuals with disabilities. See 29 U.S.C. §794d. While the Trademark Electronic Application System (TEAS) forms do comply with Section 508, the form previews currently do not meet all standards for web accessibility.
